Your Privacy is Important to Us!
Dr. Dial does not accept insurance assignment. This means that your treatment with Dr. Dial is not part of your permanent medical record with your insurance company, unless you choose to notify them. In addition, Dr. Dial is not restricted to a set allowable length of session or a set allowable number of visits that may be indicated by your insurance provider. Payment is made directly to Deanna L. Dial, D.O. at the time of service.
If you would like to submit your charges to your insurance provider for reimbursement, a detailed billing statement can be generated at your request. Dr. Dial will be considered an "out of network" provider.
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
- What is my health insurance coverage and benefits?
- What is my deductible and has it been met?
- Do I have a co-pay? If so, how much is it?
- What is my wellness visit coverage?
Policies and Rates
Payment is due at the time of service unless other arrangements have been made in advance. Dr. Dial accepts cash, credit cards, debit cards and checks. Overdue accounts may be referred to collection agencies as a last resort. There is a $35 charge for unpaid returned checks.

Adult, 50 min $500.00
99215 Medication re-evaluation with psychotherapy, 60 min $375.00
99214 Medication re-evaluation with therapy, 45 min $300.00
99213 Medication re-evaluation with supportive therapy, 25 min $ 200.00
90846 Family Therapy without patient, 50 min $400.00
99442 Telephone Consultation, 1 to 15 minutes $100.00
99443 Telephone Consultation 16-30 minutes $200.00
99444 Telephone Consultation 30-60 minutes 350.00
00000 Court Appearances, $600.00 per hour, retainer required
90792 Urgent Telepsych Evals, $650.00
90792 Urgent Evaluations $650.00
81404 Genesight Buccal Swab Testing $50.00
00000 Paperwork associated with your care/court/FMLA/disability per hour $300
